Feedback and Complaints

At Mai-Wel, we welcome feedback and take all concerns seriously. This page is a single place to share feedback, suggestions, complaints, anonymous disclosures or use our external whistleblowing service.

Complaints and feedback provide the opportunity for us to improve service delivery or recognise the efforts of staff in the delivery of quality service supports or interventions.

You do not need to know the “right” category. If you are unsure, start with the option that feels closest. We will help guide it to the right pathway.

Submit Feedback, Suggestions or Complaints

If your message is feedback, a suggestion, or a complaint about a service, you can submit it through our feedback and complaints forms. Your feedback also helps us recognise what is working well and where we can improve.

Whistleblowing: Report Wrongdoing Confidentially

If you have serious concerns about misconduct or improper behaviour at Mai-Wel, you can make a confidential report through our independent external Whistleblower service.

This service is available to support people who want to report concerns safely. You can choose to provide your details, or you may be able to report anonymously.

Use this option if your concern relates to serious wrongdoing, such as:

  • Fraud, theft or corruption
  • Serious breaches of law or policy
  • Serious misconduct or unethical behaviour
  • Risks to people’s safety, wellbeing or rights
  • Conflicts of interest that are not being managed appropriately
  • Victimisation of someone who has raised a concern
